ALL FORMS AND QUESTIONNAIRES FOR CLASS USE

Dear Teachers,

Please note it is vital to make sure that students are satisfied and have the means to give the school and yourselves adequate feedback on your lessons. Thus we have provided you and the students with a variety of evaluation forms.

However, it is your responsibility to choose the correct ones and/or those forms which you feel are suitable to use, then to make sure that both yourself and the students each have a copy to fill out before the fall and spring terms end in March and September.

Next before you hand out reports to students, make a copy for the school's use, then either we will send on to students or you can give them directly, as you prefer.

N.B. Your bonus-depending on satisfactory circumstances- also requires that this activity be completed punctually.

The next 2 forms are for your reference when writing all reports:

The following forms should be used or noted at the start of a new class or student:

Demo lesson attendance form: For all new visitors to class or interviews!
Basic sentences used in class Helpful activity to use in first month of class!
Student skills evaluation report Hand out once a year or when quitting.
Student skills evaluation form Write after 1 month of student joining and write again end of term to chart students skills progress
Student's first questionnaire about class: Students to return after 1 month only!
Regular class progress questionnaire: To be done every 6 months, in March & Sep.
Common complaints by students: For students, but make sure you read it too
Student self assessment fluency questionnaire: The aim is for the student to rate their speaking confidence/free conversational skills each week.
Final Questionaire (when quitting): Please make sure you give this questionnaire when students quit!!
